Terms

Your responsibility

  • You must keep up-to-date with all appointments and treatment recommended.

  • You agree to pay your membership on time.

  • You agree to provide the clinic with at least three months’ notice if you wish to cancel your membership.

  • If you cancel within the first 12 months of commencing the plan, you will be required to repay any benefits you have received, pro-rata.

  • You must follow your recommended oral hygiene regime at home.

  • You will provide the required notice if you wish to cancel or reschedule your appointments.

Terms and conditions

  • The provision of consultations and treatment is subject to receipt of the consecutive monthly payments.

  • The Worldwide Dental Emergency Assistance Scheme is a scheme established to offer support and assistance to dental plan patients who request treatment following a dental trauma and/or dental emergency or oral cancer. The Scheme responds to requests for assistance on a wholly discretionary basis. This means that, whilst the Scheme aims to provide benefits in most cases, it has no obligation to provide any benefit unless it first decides (in its sole and absolute discretion) that it should provide a benefit. There are some circumstances in which the Scheme is not designed to help and these are explained in more detail in the Scheme Handbook. Each request will be looked at individually to assess the request for benefit. Certain restrictions and limitations may apply in the event that the request is accepted and it is possible that the Scheme will decide to provide no benefit.

  • An initial administration fee of £9.50 will be collected with your first payment only.

  • Discounts only apply to treatments carried out at the practice and are not included in any special offers.

  • If permanent treatment has been recommended and declined, then temporary treatment or alternative options will only be provided on one further occasion. Following this, treatment will be charged at the full rate less the appropriate discount.

  • You must be dentally fit before inclusive treatment cover is provided.

  • Teeth with an uncertain prognosis may not be covered for the first two years of membership.

  • We may not cover teeth in the event of neglect or certain incidents of accidental damage.

  • The annual membership is non-refundable.
